*Every participant must sign this waiver before boarding the vessel.

1. ASSUMPTION OF RISK

I, the undersigned, understand that participating in a fishing charter or eco-tour involves inherent risks, including but not limited to:

    •    Slippery decks, trip hazards, and potential falls

    •    Sea sickness, sun exposure, and unpredictable weather conditions

    •    Risks associated with fishing equipment, hooks, and marine life

    •    The possibility of injury or death from boating accidents, drowning, or encounters with wildlife

I acknowledge that Fish Masters Charters & Eco-Tours, its owners, employees, and affiliates have taken reasonable measures to ensure my safety, but they cannot eliminate all risks associated with these activities.

2. RELEASE OF LIABILITY & WAIVER OF CLAIMS

In consideration of being allowed to participate in a charter trip or eco-tour, I voluntarily agree to the following:

    •    I assume full responsibility for any risk of injury, property damage, or death while participating in any activities related to Fish Masters Charters & Eco-Tours.

    •    I release, discharge, and hold harmless Fish Masters Charters & Eco-Tours, its owners, captains, crew, and affiliates from any liability, claims, demands, or legal actions arising out of injury, damage, or loss, whether caused by negligence or otherwise.

    •    I understand that I am responsible for my personal belongings, and Fish Masters Charters & Eco-Tours is not liable for lost or damaged items.

    •    I confirm that I am physically able to participate in the activities and do not have any medical conditions that could be aggravated by this experience.

3. SAFETY & CONDUCT AGREEMENT

I agree to:

    •    Follow all instructions given by the captain and crew.

    •    Wear a U.S. Coast Guard-approved life jacket when instructed.

    •    Refrain from reckless or dangerous behavior that could endanger myself, other passengers, or the vessel.

    •    Not bring prohibited items such as illegal drugs, excessive alcohol, or weapons on board.

    •    Understand that the captain has the final authority on all matters related to safety and trip operations.

4. MEDICAL EMERGENCY AUTHORIZATION

In the event of a medical emergency, I authorize Fish Masters Charters & Eco-Tours to administer first aid and, if necessary, arrange transportation for emergency medical treatment. I understand that I am financially responsible for any medical care required.

5. PHOTO & VIDEO RELEASE (OPTIONAL)

I  ☐ DO,  ☐  DO NOT authorize Fish Masters Charters & Eco-Tours to use photos or videos taken during my trip for promotional or marketing purposes.

6. MINOR PARTICIPANTS (UNDER 18)

A parent or legal guardian must sign for any participant under the age of 18. I certify that I am the parent/guardian of the minor(s) listed below and consent to their participation.
